It is cited here for one narrow reason: it is the only regulatory-grade source that states a verified tirzepatide injection frequency.

Outside those approved indications, tirzepatide is also supplied and studied as a research compound, sold in lyophilized vials rather than as the approved pre-filled pen product. Why once weekly, not daily

Tirzepatide is a long-acting GIP/GLP-1 receptor agonist with an extended half-life, which is what allows a once-weekly schedule to sustain an effect between injections. Frequency and the reconstitution math

Injection frequency answers a different question than reconstitution math does. Frequency tells you how many times per period a draw comes out of the vial. Reconstitution math tells you the concentration of what's in the vial, and therefore the volume, and syringe units, for any single draw. The two combine to answer a third, more practical question: how many weeks will one vial last at a given frequency?

Concentration is set once, at the moment bacteriostatic water is added:

Concentration = vial mass (mg) ÷ water added (mL)

Worked example: vial life at a weekly frequency

Setup. A 30 mg tirzepatide vial reconstituted with 3 mL of bacteriostatic water.

Step 1 — concentration. 30 mg ÷ 3 mL = 10 mg/mL.

Step 2 — volume per dose. At a 5 mg dose, volume = 5 mg ÷ 10 mg/mL = 0.5 mL.

Step 3 — syringe units. On a U-100 insulin syringe, 1 mL equals 100 units, so 0.5 mL × 100 = 50 units per injection.

Step 4 — vial life at a once-weekly frequency. The 30 mg vial holds 30 ÷ 5 = 6 full doses. At one injection per week, that vial supplies 6 weeks before a new vial is needed.

Change any one input and the vial-life number changes with it. A 15 mg vial at the same 10 mg/mL concentration holds 15 ÷ 5 = 3 doses — 3 weeks at a weekly frequency. A smaller 2.5 mg dose drawn from that same 30 mg vial (0.25 mL, or 25 units) stretches the vial to 30 ÷ 2.5 = 12 doses — 12 weeks. Frequency and dose size both draw down the same fixed pool of total mg in the vial; neither one changes the concentration itself.

The labeled titration schedule changes dose, not frequency

The approved label's titration schedule raises the dose over time, starting low and stepping upward at set intervals, while the injection frequency itself stays fixed at once weekly throughout. That's a useful distinction for reading any tirzepatide protocol: a dose change and a frequency change are two different variables, and the approved label only moves one of them. Doses per vial recalculates every time the dose changes, even though the once-weekly frequency and the vial's concentration don't move. Common mistakes

  • Assuming a research vial's schedule matches the approved product's frequency exactly. The once-weekly pattern is consistent, but a research vial's concentration and total mg depend on how it was reconstituted, not on the approved pen product's fixed cartridge.
  • Confusing a titration step with a frequency change. The label's dose increases happen on the same weekly schedule throughout — frequency doesn't change, only the dose does.
