How Many Units of Peptide to Draw on an Insulin Syringe

An insulin syringe measures volume, not dose. The number of units you draw for a given peptide dose depends entirely on how concentrated your reconstituted vial is — the syringe itself never changes. This reference is written for research and educational purposes only and is not medical advice.
The one fact that makes the conversion work
A standard U-100 insulin syringe holds 100 units per 1 mL. That ratio is fixed:
- 1 mL = 100 units
- 1 unit = 0.01 mL
That's the entire syringe side of the problem. Everything else — how many units correspond to your target dose — comes from the concentration of the vial you mixed, not from the peptide itself.
Step 1 — Know your concentration
Concentration is peptide mass divided by the volume of bacteriostatic water added:
- 5 mg in 2 mL = 2.5 mg/mL = 2,500 mcg/mL
- 10 mg in 2 mL = 5 mg/mL = 5,000 mcg/mL
- 10 mg in 1 mL = 10 mg/mL = 10,000 mcg/mL

Step 2 — Convert concentration to units
Divide the concentration by 100 to get mcg per unit, since each unit is 1/100th of a mL:
mcg per unit = concentration (mcg/mL) ÷ 100
- 2,500 mcg/mL ÷ 100 = 25 mcg per unit
- 5,000 mcg/mL ÷ 100 = 50 mcg per unit
- 10,000 mcg/mL ÷ 100 = 100 mcg per unit
Step 3 — Convert your dose to units
Units to draw = target dose (mcg) ÷ mcg per unit
Worked example. A 10 mg vial reconstituted with 2 mL of water is 5,000 mcg/mL, or 50 mcg per unit. A 250 mcg dose is 250 ÷ 50 = 5 units. A 500 mcg dose from the same vial is 10 units.
Worked example, different concentration. The same 250 mcg dose from a 5 mg vial in 2 mL (2,500 mcg/mL, 25 mcg per unit) is 250 ÷ 25 = 10 units — double the units for the same dose, because the vial is half as concentrated.
That's the reason a units-to-draw number can't be looked up as a constant per peptide: it moves with whatever concentration you mixed to. Reading unit marks on the syringe
U-100 insulin syringes are commonly sold in three barrel sizes, all using the same 100-units-per-mL scale:

A smaller-barrel syringe with finer markings makes a low unit count easier to read accurately. If your calculated dose lands on an odd number like 7 units on a 1 mL syringe marked in 2-unit increments, a 0.3 mL or 0.5 mL syringe — or adjusting the reconstitution volume so the dose lands on a marked line — removes the guesswork.
Common mistakes
- Assuming a unit count carries over between peptides or vials. It only holds for one specific concentration.
- Mixing up mg and mcg before converting — 1 mg equals 1,000 mcg, and this is the single most common source of a 1,000x error in a unit calculation.
- Reading units on the wrong barrel scale. A syringe marked in 2-unit increments can make a 5-unit or 7-unit dose easy to misplace on the barrel; use a finer-scale syringe when the count is low.
- Forgetting that "units" always refers to the U-100 scale unless the syringe is explicitly labeled otherwise (U-40 syringes exist for veterinary insulin and use a different ratio).
